Did they write the date they installed on the plug with a sharpie? Just check the date...

I have zero ideas, they seem way over due and perhaps once it reached a certain point of wear it rapidly accelerated.

Either way I'd toss them and move on haha.

Any ideas why driver side wore down badly and passenger is ok? Can I pull o2 sensor info with a Tactrix or OBD2?

Regardless of what it is, the best way ti figure out if it is an issue is to throw on a set of six and then check it again in 5,000 miles.
If no uneven wear - no issue. Probably an incomplete job.
If uneven after 5K, then you do have an issue.

Been thinking this from the beginning. Check the plug wires and coil packs. Apparently the 2GR has known issues here.

    https://www.motorreviewer.com/engine.php?engine_id=129

"Like the 2GR-FSE, the 2GR-FKS has a combined direct injection and port fuel injection (Toyota's D-4S)"

What? Two types means 2 delivery systems to potentially fail. I suppose this is an Atkinson feature? dunno.

    But more importantly and likely:

The 2GR family has a common problem with unreliable water pump and ignition coils. In some cases, they don't last longer than 30-50k miles, and that is very annoying. The 2GR-FE has longevity about 200,000 miles (300,000 km). The 2GR-FSE and 2GR-FKS engines are more technologically advanced, powerful, and fuel efficient but less reliable.

No codes or pending codes on the OBD2, and all O2 and catalytic sensors within limits. I think I'll just run it until it pulls a code.

    LS1 swap with nitros.
Perhaps this is why the recommended change interval is 60k miles.

You would have a case if bank 1 side was in the same state

Something I didn't see called out but may have missed it - apologies if I did.

There is speculation if the plugs are factory or replaced by a previous owner. Looking back at the OP's pics in the first post they have additional spark points on the side. I haven't pulled mine so I'm not sure what comes from the factory but I'm betting those plugs are not original.

And to the point that one bank was replaced earlier than the other bank we have speculated that the less worn bank was replaced and the worn bank was omitted. I think something opposite may have occurred. Quite possibly the worn bank was replaced earlier due to an issue and then the less worn bank was replaced later to catch it up on maintenance. Either way one side has more miles on it than the other one or one side is burning much hotter than the other side.

This the the first plug swap, per post #1, and OP is the original owner, per post # 8.

I recall reading somewhere or maybe in a video on the web, that the factory-installed OEM Denso spark plugs have mark (letter) stamped on the tip of each top terminal: replacement plugs you buy do not. When I replaced the plugs in my my truck at 125,000 they were OEM Denso with no marks on them and therefore not the factory installed OEMs. I later learned from a service record, that the plugs were dealer-replaced at 15,000 miles: with Denso. I'll see if I can find the video.

This is the first plug swap from this owner who is not the original - you are correct. But we don't know what the previous owner/s did to it.
What we do know is that these are not stock plugs so at some point before this owner, someone else replaced the plugs.
We don't know when (mileage or date)
We don't know why - could have just been time or upgrading to cheap counterfeit chinesium iridium plugs for the gains.
We don't know if they were all done at the same time
We don't know the the Prev-Owner was chasing a problem too.

    Again, see post #8.
Or, post #27, where he had to repeat himself as being the original owner.

My bad - I thought he said he wasn't the original owner.
So now how did the non-stock / non OEM plugs get in there?

OP - Do you have a Carbon Monoxide detector in your house?

While we are on the topic of spark plugs, how many of you are getting OEM Denso plugs from the dealership versus, say, Denso iridium-TT plugs on Amazon? I am at 95K miles and this thread is making me think its about time.

Since this is an inconvenient job that doesn't need to be done often I wasn't going to worry about counterfeit crap parts, just went to the local dealer for them.
