Truck started rattling/knocking coming from under engine, sound goes faster as I accelerate. Also sometimes some big puffs of blue smoke coming out of the exhaust. Always did my oil change full synthetic 0w-20 premium every 5000 miles.

Wow that's some pretty loud engine knock. Did you check the oil level? Is the oil filter the correct filter? It doesn't sound fuel/fuel mixture/timing related to me. It sounds more metallic like when you have a blocked oil passage(s), worn bearings, worn piston rings, or camshaft. How many miles are on the engine? I'm guessing no engine problems prior to this.

I'm thinking of the process I would go through to track this down. I'm assuming no "low oil pressure" or check engine warning lights are on. I would first check the oil level and condition. If you're able, check the oil pressure to make sure it's within spec: https://www.tacomaworld.com/threads/oil-pressure-measurement.716910/. Also check that the oil filter housing is still in place/not grossly leaking.

If you have a scan tool definitely run that to see if you're throwing any error codes. The blue smoke from the exhaust seems like you're getting some oil in the combustion chambers. You can pull the plugs and check the condition of them/see if any are oily. Checking the engine with a stethoscope may help you pin down the location the knock is coming from which may help zero in on the problem.

sounds like a rod knock to me, but to be certain...
Hard to explain but increase rpm & then quickly repetitively snap the throttle & the raspy deep knock should worsen & sound more aggravated by snapping the throttle..

Its an art & definitely a tail tell sign if its a rod knock....

Well I did sample my oil and not too good news. Some sparkly metal particles floating in the oil indicates my piston bearing is wearing...

    yeah, its dead.....
If you cut off the backside of the oil filter & open up the paper pleats, you will see gnarly metal debris, flakes from your rod & crank bearings....
You need an engine at this point, or a reputable machine should to do a complete rebuild on what you have...

Also, when idling I get horrible MPG and also going from a stop to accelerating i see the blue bar of gas I reset the counter and shows like 4MPG but then on the road or highway I get regular 23-25MPG which is good. Why ?

Youre beyond all that, youre engine isnt going to be efficient when its literally coming apart....

stop running it & driving it if youre going to fix it....
