Assistance diagnosing noise

    A friend of mine has a 2018 access cab 4cyl. Three summers ago, due to a negligent oil change place leaving the radiator cap off, They found themselves overheating far from civilization. They received some bad advice and drove the truck for at least 20miles while overheating.
    Since then sometimes on cold starts it makes an awful racket that sound like a dry valve train, like a diesel. as if there is no oil being pumpe, releases some smoke, noise stops and they carry on.
    She has taken it to the dealer twice and they have no idea what's going on. They told her it needed an oil change the first time and it was silent for a while after that. Of course, can't get it to replicate while the dealer has it.
    Anyway, second trip to the dealer and still nothing concrete and she asked for just a diagnostic. They mentioned possible cracked manifold issue. Nothing they say makes any sense.
    Well she just sent me another video of the noise and its totally changed. My first though is it sound like an impact drive going HAM. thinking through it a bit more, it almost sounds like the starter is trying to start? is this possible?

    Sounds like a bad headgasket is leaking a bit of coolant into the cylinder(s) while it sits. When the engine starts again it "chokes" on the liquid coolant in the cylinder for a couple seconds, then it runs better. The smoke is the coolant steam coming out the exhaust

    Engines can't compress water so it does indeed sound like a diesel until the liquid gets out of there. Now the sound is worse because damage has been done

    Does the coolant level drop at all?

    yeah, thats a really weird noise but then again, we have no idea of throttle position during the recording...
    she could be hammering down on the throttle...
    the hood isnt even up.....

    thats bit of a stretch.....
    I get your theory but ive never heard any head gaskets on the verge of hydrlocking make that much noise....
    maybe a lazy timing chain tensioner if it has a chain due to oiling issues, sludge, etc....?
    early stages of front crank dampner seperation?
    Doesn’t sound like crank bearings, but I’d believe that over head gasket…
    could even be cat converter internals coming apart & lose in the cat shell...?
    could be anything from that video, but head gasket leaking coolant making a catastrophic noise like that aint it....

    Get a long PVC pipe and CAREFULLY poke around with your ear at one end of the pipe. You should be able to determine where the loudest source of the noise is.

    sounds really fast for idle, maybe it was just started...
    A mechanics stethoscope is your friend here if you truly want to isolate...
    This is a hands-on deal to really know what the source is.....

    That sounds as bad as the advice given to continue to drive while overheating. Someone’s number should be blocked at the very least.

    Terrible advice.

    Wild ass guess, its the water pump chewing into the timing cover.

    But without a stethoscope its impossible to tell.

    It needs to see a qualified mechanic and left over a few days.

    Again, it sounds like had a bad head gasket for a while. Problem happened only at startup, choking on liquid coolant for a few seconds then running normal.

    Noise and strange smoke would come only come back sometimes at startup. After sitting and getting cold.

    That's why I asked about the coolant level

    The video was taken after it did that for 3 years. "Ever since" it was driven for 20 minutes while overheating it has done those symptoms..

    Now something is much louder so he decides to post. Now the noise could be anything. Im just saying from the description it really sounds like he had a slow HG leak for 3 years

    That sure sounds like the VVT rattle I hear posted on here alot. That solenoid valve has to be pretty gunked up from the over heating.

    I understand there is self cleaning fuel injector thing that same people are talking about in other forums. Any thoughts on that? I recieved video of the coolant level with the cap removed. I’ll post it up here shortly. It’s not clear pink by any means but not seeing oil

    Not applicable to 4 cylinder engine. This engine only has port side injectors
