    And what is your protocol on LR? I have never trained on it, and the county I work in doesn't have it.

    EMRs basically provide basic life support. Drive the ambulance. And we can give ASA, Oral glucose, and assist with nitro.

    If it is likely they will be giving blood you want to start LR. We always started LR on all trauma patients but sadly now we don't carry it anymore and are left with NS. We were the last ones around to stop using it. Of course the way shit changes around the US now we may have it back next week, next month, or tomorrow. Who the hell knows?

    I really liked it when my state started certifying EMTs with more training as it freed me up from having to do all of the relatively simple tasks such as starting IVs and allowed me to concentrate more on patient assessment and care. Also, in most rural areas, where EMS is all volunteer, people simply can't afford to move to a city and take a two-year mobile intensive care technician course. Intermediate-level EMS personnel who can do more than "scoop and swoop" can save considerable lives.

    Then have that taught at entry level emt training. I don't work on an ambulance, but I really want to. My comment was geared more to the first responder side of things.I am a ski patroller. I can assist in giving certain drugs, administer oxygen, spinal immobilization etc. But my I.D. card says its seems much easier to follorwmy level of certification in AFA. When I looked on a the Canadian red cross website the guidelines for EMR fit more into what I have than AFA. I much prefer the way the way its layed out in the U.S.

    In a city it's no problem to get trained at the highest level desired. However, in most of the states I've lived in the city is a long ways off and not everyone can go there to spend the time needed to receive advanced level training and certification right from the start. Also, not everyone will have the desire to pursue this training. It's better to get a sufficient number of people trained at a basic level then send a few interested people off to get higher-level training rather than require all to be trained at an advanced level from the start.

    Let me use an analogy from my scuba training. There were about thirty in my basic open-water course, about fifteen in the advanced course and only about six of us in the master diver course. If the college had posted a one-year master diver course as the only scuba certification offered nobody would have enrolled.

    LR is used on penetrating trauma/major bleed that will probably go to surgery.

    well...my worst nightmare of something that can get me fired...

    My other worst nightmare is coming up on a 911 call in the ghetto (most of Orlando) and some upstanding citizen shooting me because the company I work for insists we wear navy blue pants and shirts so we look just like cops :rolleyes:

    There's like 6 of them and there's only 3 of them I'd willingly take on a call. Sadly...I rarely ever work with them.

    And after working in one place and having my Sup come on a call and try to get me to give Combivent to a patient who's having minor SOB from a panic attack but who has extensive cardiac history, I'm reluctant to relive the experience.

    Then I'd lose my license for allowing her to give a drug to a patient that's contra indicated.

    I just told her no, it ain't going to happen. When she tried to give me shit at the hall I lost it on her. So she sent of the patient care report to our medical director. He came back saying the course of treatment given was correct and he wouldn't change what was done.

    So she wrote me up for swearing at her instead...
