Crawled under my truck for a minute and noticed this. Should I be worried? What should I do about it. I'm pretty sure I shouldn't have rust on my crossmember image.jpg

Clean it up and repaint it.

And you should probably get that dirt out of the cross member also and anywhere else it's hiding. Dirt gets wet, holds water against your undercarriage and encourages rust.

I second what @DCSBFTMFW said. You definitely wanna get that dirt out of there... What's on the outside doesn't matter nearly as much as what's on the inside- they tend to rust from the inside out.
Would clean it all out and paint it.

Grind the rust off with a wire wheel until the rust is gone, and touch up with Rustoleum, or equivalent.

POR-15, the stuff is amazing. I used it on the undercarriage and intercooler brackets on my Cobra 10 years ago and it is still like the day I applied it. Its only weakness is UV radiation from the sun...

    From the website www.por15.com
"POR-15® Rust Preventive Coating is a high performance coating designed for application directly on rusted or seasoned metal surfaces and will stop rust permanently. POR-15® Rust Preventive Coating is non-porous and seals and protects many surfaces from water, chemicals, salt, and other corrosive contaminants and is able to be painted over rust. POR-15® Rust Preventive Coating is an effective anti-corrosive and rust preventive coating that offers superior chemical resistance due to its dense, cross-linked molecular composition and non-porous attributes.

The secret lies in POR-15® Rust Preventive Coating's curing process. Most paints dry through evaporation, but POR-15® Rust Preventive Coating dries faster when moisture is present. It has the opposite chemistry of ordinary paints. Now add to that POR-15® Rust Preventive Coating incredible hardness, toughness, and flexibility and you have a coating that is practically indestructible. POR-15® Rust Preventive Coating is a permanent rust repair great for auto frame and chassis restoration, protecting valuable equipment against rust and corrosion. You'll be amazed at the toughness!"

as delicate as these frames are you will have to take extra care of them. I wouldn't off road them, or even take them in the rain.
