I ended up pairing this with a BESIGN BK01 Bluetooth adapter (using a female to female 3.5mm adapter) and now have Bluetooth on my OEM head unit.
So far the audio quality of the combined adapter and BESIGN seem really good. The attached USB slot appears to be designed for reading SD cards or USB drives for songs, and you can use the head units controls to switch between tracks. However since I'm just using it to power the Bluetooth adapter, my head unit controls do nothing other than change what track the display says I'm on.
I didn't see any specifications on the USB, so I don't know what kind of power it is able to provide. But it seems to be able to power the Bluetooth adapter just fine, which was my main concern.
I will come back and update my review if anything changes.

Do not try and pull the "T shaped" center dash cluster away from the dash, it will break. Remove the glovebox and access the back of the radio from the glovebox opening. If you had a Toyota CD player it will no longer work, since the Radio has to be set for CD input for GROM Aux input to work, but who uses CD's these days?
It seemed like a better solution than those fm transmitters. But it wasn't. It makes an annoying buzz. (I seem to remember reading others having this problem). The buzz makes this solution not worth it.
to reduce noise the toyota audio system uses balanced audio , but 3.5mm jacks are not balanced. to prevent noise the device would need to have a seperate analog ground.
as it seems it did. upon taking it apart, the ground from the 3.5mm jack was simply connected to the wrong point, to the main ground, not the analog ground. the ground should be connected closest on the connector inside to the other two wires. I just unplugged the JST XH connector, removed the ground pin with a screwdriver, and put it in the third pin in that connector, according to the labels on the back of the PCB. with that modification, it sounds just as it should without any interference.
to be clear it may be a fluke that mine had this incorrect and everybody has theirs correct, but if somebody has this problem I hope this helps.
