After having an oil change done on the weekend I noticed that my driver's side CV boot that covers the bearing is loose and so the bearing is exposed. I had both sides changed and all the boots for the first time in october with 176000km on my taco. I have around 191000km now. It probably came loose more recently driving through deep snow but shouldn't be. I'm worried i'll need a new bearing. I'm hoping just a new boot and clean / regrease the old one.

If the bearing was exposed ( boot loose ) you have probably slung most of the grease out of the bearing. How long it has been exposed to the elements is the unknown factor along with how much dirt and grit has gotten into the joint.

Reduced grease along with dirt and grit will greatly reduce the life of the CV Joint. I think at a minimum the axle should be pulled and the CV Joint cleaned and inspected repacked and sealed with a new boot, Properly Clamped onto the axle.
