I think he bumps a button with his hip.

He seems to be working with glass....

Maybe something being printed on glass?

Someone here on TW must know what this machine is Eh ...

Looks like a machine to silk screen glass or plastic sheets.

I don't think he dies, he is just stuck in there.

Edit: At about 2:16 (or 10:44:41 top screen time) you see a reflection on the glass at the left of someone coming to help the guy.

I see many things wrong here...please pardon...I work in a factory where safety is a big deal. Whoever that company is.....is gonna get their ass whipped by OSHA. Assuming that video is actually REAL (and not setup).

#1 - SAFE BUTTON? Lock out tagoout?
#2 - Anytime you have a machine like that, it requires you use BOTH hands to turn it on (TWO start buttons) and those two buttons are usually on opposite ends of the machine.
#3 - The machine should have some sort of safety switches/proc sensors that it'll only apply FULL force if its in the full down position. Everything else during the downstroke, if it senses resistance - should pop back up.

It looked like some sort of screen printer machine. Weird...but the amount of force required to kill someone like that is extreme. All that *extreme* force supported on a HINGE? Unless that thing was hydraulic in nature....??

This reminds me of a guy who got killed in our plant in our pressroom. I think OSHA found some of the safety devices were defeated and the guy was doing something he shouldn't have with these device 'running'.

100% agree with you Jan. That machine is lacking two key things: Anti-tie down switches and a pressure detection. The Anti-tie down requires two hands to start the machine. The pressure detection would reverse the clamping mechanism to the open position should the pressure exceed a set point.

That machine needs some work and the operator needs some training.

He's probably doing routine maintenance - something that he's done many times before.

If you watch the hinge come down, you'll notice the slider thing moves across from bottom to top. While the hinge goes back up, that slider thing moves back (from top to bottom). Assuming the 'bottom' is its HOME position.

Just prior to all this happening, it appears he hits a button or something on the front of the machine..... and the slider thing stays up at the top.

Then, he gets in there....lays down and does one thing. Gets out....gets the roll of tape....and lays in there again.

So - Do we really know the entire story?? Why didn't it crush him the first time he was laying in there? Did he press a stop safe button before getting in there? Was the safe button faulty? Did something malfunction? It coiuld be anything.
