sr5 acc cab

My 04 with 350K on it suddenly started failing and running very rich. 5 out of 6 of the plugs are loaded with black carbon. I have the injectors out and will test them but I doubt 5 would go bad at once. I've read that the fuel press sens can be suspect. Is there a possibility that the timing belt has jumped or the computer is OTL?
