I was able to configure this truck on the Toyota website:

    2016 Tacoma TRD off-road

    4x4, 3.5L V6 Engine, 6-Speed Manual Transmission
    Double Cab, 5-ft. Short Bed

However, searching on Edmunds, I cant find any trucks like that with 200 miles (SF Bay Area). Is this a configuration that no dealers are interested in carrying?

Here is one on cars.com. Pretty easy to filter them by color/transmission/year etc. The only bad part is no manual for long bed, but you want a short bed so you're golden.

Thanks for suggesting cars.com. The nearest manual transmission is 500 miles away :(

Keep in mind not all Tacomas are posted on that website. You're better off having a dealer locate you one. Might be one much closer to you than you think. Possible they could do a swap for you.

The sales guy claimed that the TRD off-road 4WD manual models are only put there "for show" on the toyota website. He was completely un-interested in working with me.

Go to another dealer. If Toyota makes one, a competent dealer can order you one.

About 1/2 of dealer salespeople know jack shit. At the end of the the test drive on the 2016, the sales guy was asking me questions about the 2016. Not the other way around.

They're lazy as well. Toyota vehicles, especially Tacomas, sell themselves so they have no incentive to bend over backwards for unusual requests.

If he isn't interested in helped you. You shouldn't be interested in giving him sales commission. Move on to the next salesman or dealer.
