I have a 2005 DCLB with the SR5 package. Recently one of the rear window switches stopped working, the window will open and close from the driver's master switch, but not the one on the door.

Any suggestions on what might be wrong or the part numbers for a new switch?

The things I would check is to verify if your getting power to the door window switch itself, if so then it's your switch most likely. If not, then your problem is elsewhere.

Any tips on testing and taking out the switch...

The switch panel just pops off, use a tester on the switch to see if it's getting power. Could be a "light bulb" tester or ohm reader if I'm not wrong. I'm not by any means a expert with electronics so this is just what I would try to do to see if it's at least getting power. I would Google it or wait for some one who knows more about this stuff then I do to reply if you aren't sure still.
