Tonneau with Ladder / Lumber rack opinion anyone?

Hello,
I'm looking at adding a hard folding Tonneau cover with a ladder or lumber rack to my Tacoma and looking for recommendations and/or photos of setups. Also, does anyone have an opinion of a Tonneau cover vs. a shell? I have a shell now, but I don't like the lack of headroom it provides and haven't found a good ladder/lumber solution to my setup since the shell overhangs the edge of the truck bed.
