At the dealer again, needs 2nd set of rotors (heat spots and wear) and 3rd set of new pads again at 18k miles. Service manager says everything checks out ok including calipers and check valves. All I can get out of them is 15% off of repairs, and a recommendation I call Toyota to try to get part of the $400 this is costing refunded. I have them using Napa (better) pads that cost $40 more.

Mileage don't mean squat when it comes to brake wear. Like another poster said if most of your driving is city then your pads will wear quicker then a dude who does 90% highway.

I saw you said traffic engineers own the brake shops.

I'm guessing that means you do a lot of stop and go.

How is the brake wear on your Tacoma when compared with other vehicles you've owned & driven under the same circumstances?

And the next crazy question how is your fuel milage?

Something is wrong as others have mentioned or you ride your brakes. My '09 had about 40% left of the stock pads when I sold it at 50k miles. My '14 has had no abnormal brake wear and I have 24k on her.

    ^^^^ THIS.

My work, my home, my grocery store, my costco, my mall, my kids school and other kids daycare are all within a 3 mile radius. Only see interstate travel on weekends if going somewhere.

After 5.5 years and about 55K miles on my truck I had 4mm left on the factory front pads.

Sounds like something is wrong on the OP's truck... with so many issues in such a short time span.

If you aren't resting your foot on the brake, then you have a mechanical issue with the brake system. If the dealer will not help I would escalate to Toyota Corporate. It should already be documented in your service history.

    Big questions here:

Are both sides worn evenly more or less?

What does you gas mileage average?
