Had mine checked today. They also say they did a software update on the transmission. 2.5 quarts low.

I have 2,300 miles and mine seems to be getting worse. I had a 3rd gear shift flare the other morning as well. I'll take it in soon.

One thing you can do to mitigate this is when you are backing up is to click it into Neutral while you're rolling back the last couple of seconds. It seems to allow time for the fluid to fill an accumator/shift circuit or something, so when you shift into D, it normally drops in right away.

According to other posters in that thread the op code is just fluid check up not an actual tranny software update

Correct. I honestly think all they did was reset the learning mode on the transmission. It feels like when it was new and needs to be broken in again as illogical as that sounds. It did shift a lot smoother when I was driving.

Funny thing, just broke 1600kms (1000 miles) and it is totally acting different now, shift points have changed and it feels more powerful. Really reminiscent of my Gen 2. Don't know if that's the sweet spot, but it seems to pull off the line with more confidence. I'm light footed, so before it felt a little dogged, but today was a lot different (or its all in my head).

Mine took around 3000 miles to start feeling good and seeing way better fuel mileage. Before 3k i was getting 16.4mpg at best on the highway. Seen a steady increase since then.

After my first cold start this morning, there was no delay between R/D. I drove about 45-59 miles today. Everything has seemed very smooth, actually all the shifting seems more fluid and natural than the first 1400 miles i put on the truck.
    Keeping my fingers crossed.

This is exactly what is happening to me as well. I took about a 100 mile round trip today and the whole time I was fighting it sticking in 4th gear on the interstate 60-75 and not wanting to shift out of it. Dealer checked my fluid a few days ago and added almost a quart and that seemed to fix my hesitation when cold in R & D so far but not the 4th gear stick. Going to drive it a few more days and see if it gets any better. I've got about 850 miles on it currently.

It's common for the tranny to stay out of overdrive until the engine warms up a bit, but if it's not shifting out of 4th on a 100 mile trip, then that doesn't seem right to me. Was ECT on?

Glad your R to D is better. Fluid was the fix for mine too, bet never had the 4th gear issue.

I haven't touched ECT yet. It'll stay there for awhile, even if I'm off the gas completely and going down a hill. It will eventually shift but seems to hold it significantly longer than it should. I'm just hoping it either works itself out or they can find the problem/come out with a fix for it. I haven't towed with it yet but am curious to see what it tows like if it drives like this with no load behind. Right now it drives more like a 4 speed than a 6 speed ha.

Here's a vid of mine getting stuck in 4th. I've had the dealer add 1.5 qt this has got to be a shifting issue.

Exact same that mine has been doing. Nice catch on video!

When does this happen . Looks fairly cool where you live and may have something to do with sticking in fourth .
You will not get overdrive when the trucks transmission and engine are cool . Once the 2 transmission temperature sensors on the valve body are warm enough , they communicate with the ecm and will allow overdrive .
With this said , if the transmission is only sticking in fourth during your initial warm up , the transmission is working properly .
If the transmission is sticking in fourth gear after the transmission is completely warmed up , the problem can be verified easily with a scanner
Sorry , jumped the gun here . I just read the above posts .
    Get the transmission scanned .

The truck wasn't warm however this stretch of road it always happens even with the truck completely warm. Truck is going on a very slight incline I will let off the gas as seen in the video and it'll stay above 2k RPM even tapping the gas pedal or hitting the brake will not have the truck upshift. It seems to always happen on an incline. Until the road levels out I hit the gas pedal again then it up shifts.

And my guess or assumption is that you still have a low fluid level causing the transmission to stick in fourth .
The quickest way to figure this out is by having a toyota technician take your truck for a drive making sure the transmission is warm and the scanner must be connected when he does the road test . Verify the 2 transmission temp sensors are in contact with warm oil . Both sensors will be reading very close to the same temperature if the fluid is at the correct level . If the fluid is still low , there is a good chance 1 temp sensor will be submerged in warm fluid while the other sensor is still showing a cool reading . If there are no codes showing when the scanner is connected , temperature is the first thing you look at when there are problems obtaining or holding overdrive .
Either you are still low or there is damage done to the valve body from running with a low transmission fluid condition that caused the sticky shift .
Needs to be diagnosed properly of-course ....Without a proper diagnosis , this is only an assumption , however transmission damage from running low on fluid was a serious problem with the 2nd gen transmission .
