    Forgot to mention warped rotors or drums the pedal will pulsate, if steering wheel shakes its more likely the front discs. This is with the presumption your wheels are not out of balance or if you had any suspension work done that everything is correct an tight. 40-45 mph is about the right speed for unbalanced tires to start to wobble. Check it again without using the brakes, if it does I doubt its the brakes.

    I have had "the shakes" since I bought the truck new in 2013. I've had new tires, balance, and alignment and it still does it intermittently, as it always has.

    While the brakes are applied? I've had this same issue since I bought the truck. But it's ever now and again. Both with stock and new wheels. Brought it to Toyota while it was still under warranty and they did a full brake job. When away for a bit but it's still there.

    Have you had your wheel bearings checked? They may be out of spec

    My apologies, I should have been clearer. No, my steering shakes intermittently just as I drive down the road. It just seems to come and go.
