    This same thing happened to me last night. Reversing down my driveway which has a hill. In reverse, clutch in, foot on the brake. Brake pedal went to the floor, truck started rolling back un controllably, pumping the brake with no effect. I yanked on the e brake and it stopped the truck just before hitting the wall. There was some grinding or chattering noise. My girl friend came running out thinking I hit the dog or something.

    I tried to replicate it but couldnt.

    I will call the dealership today, but I dont have much faith in the bozos at the local Toyota dealership.

    I have yet to experience the problem again. Neither Toyota or I am sure what caused it. I tried to duplicate it numerous times with no luck.
    Good luck with Toyota. While they took it very seriously, they never really did anything as they couldn't replicate the problem.

    Alright, this is the interesting clue. I've gotten this noise three times now. The first time scared the hell out of me, but I learned how to make it do it after the second time. My garage has a small lip that you have to drive over. Sometimes I don't have enough speed/gas to go over it as I guide the truck between the other vehicles and try to keep it close to the wall. When I hit the lip and the truck didn't go over and slid back, the grinding/chattering occurred. When this happens, the gas pedal DOESN'T work. I have to let the truck roll back a bit and apply the brake then move forward again. Possibly related?

    Cant say if the gas pedal reved the engine or not. I had my feet on the clutch and brake.
    Is the sound you hear like a heat shield rattling, or like the engine is being 'lugged' from being loaded with no power to deliver at low RPM?

    The sound I get is definitely a grinding. You can hear/feel it, even with the windows up. Heat shield rattle is a rapid "TING TING TING". Lugging is obvious if you have driven a manual before (15 years of it here). This didn't sound like either. It sounds like gears grinding together, or a being forced in the wrong direction. I seem to remember that it sounded like it was from up front, but that could have been reverbing back from the garage.

    That chattering noise has happened to me about three times. Each time was when I pulled up to an decent incline and I let off the brake pedal and the truck rolled back a foot or so before I was able to hit the gas pedal. It made the chattering/ ticking noise from the engine bay. I also saw a light flash on the dash which looked like the traction control icon. I never lost the ability to brake.

    I was trying to replicate what happened this weekend but couldn't. I noticed that if I press the brake pedal, after a couple seconds the pedal goes to the floor. It doesn't take much force, doesnt feel like it building pressure the. If I pump the brake it firms up, and goes to the floor again. It just feels spongy like brakes that need an air bubble worked out.. They stop the truck on the hill. I have never had a vehicle where the pedal goes to the floor over and over. Granted this is my first vehicle with ABS and power everything. The brake fluid reservoir is filled to the max line.

    Could someone check this on their truck and let me know if im going crazy?

    Stacked spacer lift, 22x12 wheels with stretched 33's, tow mirrors, bull nuts.
    That's normal... It's the solenoid for the traction control chattering at you. It's making sure you don't spin a tire.

    This is absolutely not normal, but is the symptom of a failing master cylinder or air in the line. Either way, you need to get this looked at ASAP.

    The grinding sound is the electronic brake booster modulating fluid pressure, mine is 2nd gen but I think mechanically it's the same system, just doesn't have the crawl control software. I use the downhill descent assist on really steep hills and it grinds away like it's about to explode.

    Sounds like we are having the same issue! I started a thread last week with all the details:

    I let the tech take the truck home for a night and he experienced the issue as well, so corporate is putting some effort into fixing it. They have replaced the master cylinder on Friday, but this made no difference as I had it happen two times yesterday. I just called the dealer and they are reaching back out to corporate today for the next steps.

    Got it towed to the dealership at toyota's expense.

    They told me there is an internal master cylinder leak. They will have the part in a week. Im not sure if this explains, or will fix the brakes failing to stop the truck on the driveway though.

    Got the truck back today. They replaced the master cylinder and ABS actuator, 3k in parts and labor. Brake pedal still slowly goes to the floor. The manager couldn't explain why, and thinks that maybe it should go to the floor. He is going to jump in another truck tomorrow and let me know how those brakes feel.

    What else could they do wrong? Way over filled the brake reservoir. This is minor, but it shows the quality of this dealership.
    They also adjusted the E brake, now both rear wheels are dragging. The truck felt sluggish driving around and didn't coast very well. I jacked up the rear end and struggled to get the wheels to turn by hand. Put the laser thermometer on the brakes and the rear drums were hotter than the front disks after a drive around the block.
    Im going to get it towed back tomorrow.
