I ran into a Toyota rep a few years ago and asked him why the DCLB + manual transmission was not an option. He didn't know.
I have heard a few people on this forum say it's simply because lack of demand but I do think years of pent up demand could mean Toyota could sell a good deal of them. After driving the manual and auto, the manual sounds much more appealing but like many, we are left to compromise.
I don't think it could be terribly expensive for Toyota to equip a DCLB with this option. Most of the parts already exist in other configs.

Anyone know the REAL reason? Has Toyota ever gone on the record?

You forgot the option: "Not that I don't want you to be able to get the configuration you'd like to have, but the cab bed combo I wanted came with a manual"

Because that's too many configurations for Toyota to tend to, so yes demand.

Honestly, U.S consumers are becoming less interested in manual transmissions so it's only natural the manufacturers are reducing the availability.

Dealers don't want the unsold metal taking up space on the lot so they order models which would appeal to a wider customer base. I'll bet on average they only get 1 in 10 asking if a comparable truck is offered in a manual transmission vice the automatic.

I've heard it said that only the American prefer automatics; the rest of the world drives stick.

Does Toyota sell DCLB M/T in other markets?

Not much on mods - Leer 180, TRD wheels, Mobtown sliders, rear hitch, helper springs on the rear
I would have been happier - if Toyota had offered the Tacoma their 4 cylinder Access Cab in 2 wheel drive livery with a manual transmission - but the only way to get a row your own stick was with a four wheel drive train - which is what I have.

I occasionally need to carry two or three extra people, so as much as I would love an AC OR, a DC OR with the long bed would be the better choice for me. I guess its easier to use a bed extender once or twice a month on a short bed than it is to cram an adult into the back of an AC.

As far as I know the Tacoma is only built for the US market, everyone else gets the Hilux.

It is baffling to me you don't get your choice in all configurations between AT and MT. I am happy with what I have, but a MT 4 cylinder 4x4 DCSB or DCLB would have been interesting to me as well.

North America so US, Mexico and Canada. Although I do think you can get the Hilux AND Tacoma in Mexico.
