Weird thing is a Sienna (or a Honda Pilot) the spare tire is stored under the chassis, you can have a load in them and still be able to get the spare out and mount it.

The Ridgeline, no, you'd have to unload to get to the spare or find a way to secure it in the bed.

If you have a blowout while hauling heavy, you probably have bigger issues than spare accessibility. Like how your truck is a mangled heap in the ditch.

If it's just a nail or something in the tread, slime it, re-inflate and go on your merry way. No need to mount the spare.

Unless you are talking about some other rare scenario, like boulder crawling with a bed full of lumber. In which case you need the services of a mental health professional more than you need an accessible spare.

What if you just left home depot with a new stove in the bed and while there you ran over a nice nail or two in the parking lot. It is leaking too much for slime to hold and leaking to fast to hold air. Now what do you do?

Yah, I guess it's true for many flat tire situations a reinflation kit can do the job. Consumer Reports came up with this http://www.consumerreports.org/cro/2013/10/tire-sealants-to-fix-a-flat-tire/index.htm

A plug kit + reinflation kit is even better. I'm just not a big fan of A) donut spares and B) spares that are inaccessible with a load in the bed.
