My axle seems to be tilted up slightly.

Im thinking its caused by my severly worn and broken rear leaf springs. Everything is stock.

As your can see the bumper does not line up and everything appears to be tilted.

Also, where would be a good place for an Albertan guy on a budget to order stock replacement leafs?

yup, leaf springs are getting pretty bad....when you do the deed you should sand blast the perches and then give it a salt proof prime and paint, it will help keep the rust from eating the perch and frame leading to destruction....I suspect your spring perches are showing the same rust...as long as you are going to be under the truck, may as well inspect the whole frame and do some preventative maintenance while you are at it..

Yea i want to remove the bed and strip down the frame and reproof it. The dealerships warrenty job from 6 years ago is starting to fail

Will it be a PITA to align the new leafs with this angle?

Im guessing i will have to remove both leafs at once to correct it. Any recommended procedure? Can the leafs be replaced without lifting the axle and just supporting the frame?

Hard to tell, but sort of looks like you may even have a broken one, to the left of the backing plate.

If you are in or near to Calgary, Standens has a factory here that can make you new stock or modified leaf springs for a very fair price
