Taking off the pedal and properly lubing everything with a nice grease should get rid of the creaking noise inside the cabin for some time. It is the pivot that causes the noise inside the cabin. There are also two plastic bushings on either side of the metal bushing that the assembly pivots on. These two plastic bushings rapidly degrade when the assembly becomes dry of grease which causes the noise to become even worse. I replaced both of these plastic bushings and the metal one when I greased mine but it was probably unnecessary. Still wish someone made a metal pedal for the 2nd gen though.

Clutch squeak is still non existent! Everything has been smooth sailing since October with pedal/master cylinder being replaced.

I debated a bit and ended up going to get the TSB done on my '13. The clutch pedal feels a bit tighter now and overall happy. As i was outside the warranty period (3 years), although at only 55000k, and because I complained about it a couple of years ago, Toyota ended up initially offering 50% off, and then gave me 70% off the cost to have it replaced. Not too pleased it wasn't fully covered, but figured I would get the work done anyways.
Time will tell if it was worth it.
