So, I have been experiencing rough idling at cold start for about a week 1/2. I crank up the truck, and usually it idles normally in park, but when I shift into drive or reverse, the poor bastard is acting like it's gasping for breath. It acts like it's about to completely stall out. After I hit the throttle however, that completely goes away and drives fine. No CEL comes on during this mini episode, so I'm a bit confused as to why this keeps happening. Anybody have a clue as to what it could be?

Could be the Idle Air Control (IAC) valve.
    What motor do you have?

If you are comfortable with the task, you could remove the IAC from the throttle body and clean it. I haven't done this myself.
