2013 Tacoma Access Cab, 4 cyl, 5 lug.
Observations: Each of the 4 pads were identical. Each had the wear indicator.
Dummy me: Didn't take pics or write down the position of the indicator/squeeker.
Help: driver side/passenger side, inside (caliper piston) pad/outside pad,
top or bottom. Where in the hey do them little squeekers go? Or just use one
    per wheel? Where?
NEXT TIME I WILL TAKE PICTURES!!!!!!!!
I now have a metal to metal squeal when I put on the brakes just before the truck
stops (passenger side). Any ideas as to cause?

Finally found some answers. All 4 pads are the same. There is only 1 wear indicator per pad and it's at the bottom when sitting in the bracket. Once the caliper bracket inserts are in, the tabs need to be bent to hold the insert in place. Don't forget to put caliper lube on the pad contact points. BTW Torque specs are: Caliper slide pin bolt 65 ft/lb, caliper bracket bolts 80 ft/lb and lug nuts 80-85 ft/lb.

Those are some really good pads. Mine were worn all the way down to the point of smelling the backing plate! LOL Used a set of o'reilly's pads. They seem to work real good but already saving up for Akebono Pads for the next change. I'll be changing the rotors at the same time so I'll be looking for a "kit".
