I did the manual hub swap on my 98 4runner. I had heard you can potentially damage your Free Wheel Hub when you break a CV axle so I bought myself a spare to carry when wheeling. My buddy Jordan knows a lot more about manual hub vehicles since he owns a 1st Gen 4runner and he also did the manual hub swap on his 3rd Gen 4runner so I asked him what hub to buy. He told me to buy an Aisin FHT-019. Here's a link: https://www.amazon.com/gp/product/B015R1EF6Q/ref=oh_aui_detailpage_o04_s00?ie=UTF8&psc=1

In your picture, the FHT-019 looks exactly like the one on the left.

In the video below, if you go to play time 56:19, you'll see the original Toyota Free Hub Jordan is holding in his hand looks like the one in the right side of your picture. So, I'm guessing the person you got the truck from had to buy a replacement free hub at some point and replaced it with the Aisin FHT-019.

I bought brand new manual hubs and bodies from Toyota during my 4WD swap to replace the old ones I had. The original 1998 ones looked like the one on the right, and the brand new one looked like the one on the left.

They’re excatly the same. I’m having no issue running the smooth one. AISIN changed manufacturing methods over the 20 years since it was originally made.

Hmm, I got them off a guy as a spare set as well. This is common though with OEM parts manufacturers producing a part in one way for the year of assembly and then changing designs slightly as their own stock items. Maybe there is some patent issues? Aisin is an independent company but produces many of the components for Yota
