I hate to say this but you may find that the next one has the same thing happen. They should not pop out of the inner CV like that. Best wishes and if it happens again, you should really determine the best length axle and get one made that will operate so that doesn't happen again. :)

Quick story. My sis in law bought 17 sport that had a spacer lift on it. Few months into it she calls be saying it making some noises. I look under and person that spacer lift kit, also installed spacer kit for front diff carrier. The kicker is, the bolts holding diff carrier were torqued against the rubber bushing and not the washer. The carrier ended up sliding through the bolt on the passenger side diff/carrier was floating about. That same day she was driving up a steep driveway and had to use 4x4, the axle caught cam alignment bolt and tore just like in op post.
I ended up replacing the axle with one from autozone(it was like $80 after tax) and it worked perfect. So just FYI, if you’re trying to not sped big bucks, you can always check autozone.
Here a diagram for reference, circled in green is the missing washer post spacer kit install.

Do you know what caused the problem?
My buddy have 2.5” of kift Fox with the chaos upset arm, He has had this problem twice, your having. He thinks it has to do with the lift.
    But not sure
I have the king coils lifted about 2.75 in the front, and don’t want to have this problem.
We both have the drop down diff installed.

    Thanks for the info?

B0A62225-65A8-49FE-8E9E-BE4C97433379.jpg Here’s a picture of the drivers side shaft no excessive strain the passenger side was only a 1/2” inch difference remember I have the fox lift kit on the TRD pro. I know this a shorter shaft. Spoke with ready lift today they have never had this happen spoke with several mechanics from Toyota’s and and they believe this a manufacture defect with the shaft. Since I installed the ready lift kit the responsibility is mine. I have no problem with that. I still I am baffled it did happen. If it comes down to me having to have a custom shaft made for the drivers side it will be done. Here’s a picture with the winter studded tires on it 275/65/17 by 9” rims on it -12 offset summers are 285/70/17 same offset: hopefully it will be on the road next week. That's still pretty steep and the factory fox isn't really a lift.

You know you'll likely need to trim stuff for the 285s right?

That looks like a lot more than a 3” lift.

I measured Mine before I but in the leveling kit. The front with 16 by 8 inch rims and stock 265/70/16 tires was 36.5 front and 36.0 rear. With the 275/65/17 and leveling kit I end up with 39.5 front and and 38 15/16 “ in the rear. I thought so also when I put the winter studded tires on it. (It looked taller)

Are you measuring from the ground?

Proper measurement is from centre of hub to fender

If you look at the vehicle height on the Toyota website, the Pro is 1" higher.

I don't care what Toyota says I measured two at dealer and one I found in a parking lot at home Depot. All 3 were the same height as a stock off road
