Okay guys hopefully someone can help. I’ve got a 05 Tacoma with 251000 miles on it. It’s throwing codes p0138 and coil pack 6. I’ve changed all coil packs, injectors and ecm and the problem keeps coming back. Rough idle when first cranked but after getting up to speed it goes away and runs fine even at idle. Anyone have any suggestions.

If you have the 4.0 V6 I would suspect the head gasket. Lots of info about this problem on this site.

I think I will take #6 back out to see if it’s wet or not really hoping it’s not the head gasket. Haven’t really noticed a loss of coolant

Have you replaced the fuel pump?

Could be a lean misfire if you have the factory pump with non serviceable fuel filter

    Before tearing the engine apart...

    P0138
is the OBD-II generic code indicating the O2 sensor for bank 1 sensor 2 fails to have a lower voltage output below 1.2 volts for more than 10 seconds indicating a lack of oxygen in the exhaust stream.

Try a MAF sensor cleaning first then O2 sensor replacement as suggested by Lester Lugnut.

It’s just so intermediate I just started it and it ran fine no misfire or anything. I’m not sure on the fuel pump I can check to see if it’s been replaced.
