    Most likely your blend door is stuck. Either the actuator is shot or something big is jamming it.

    I believe Toyota calls it the Mode Control Servo.
    You might be able to access it with the glove box removed.

    THIS ^^^

    Remove and unplug the Mode Control Servo and see if the lever moves freely. If so then with the actuator removed and plugged in select the different modes and see if the actuator moves as commanded.
