they have about 3k miles on them. Here’s a pretty recent picture when I was replacing my upper ball joint.

That’s the back end, also the rust is due to the massive amounts of salt and rocks they put in the roads here in the winter. Will be treating it this summer. Previous owner didn’t for several years

Upon further inspection there is a block in the rear suspension. Does that need to be remove when I remove the spacers? And when I remove the spacers, will my current tires still fit?

In the picture of the rear it doesn’t look to me there is a block. Do you have a better picture? And what size are your tires?

I don't see a block either. At most, maybe a shim that's hard to see under all the smegma. Honestly it looks like the original leaf pack to me.
My thoughts on the rust in general...

Looks like something has been leaking on that rear axle. I would check the diff breather back there to make sure it hasn't rusted solid. The rear axle needs to be able to release pressure when the gear oil warms up. A new breather is really cheap and can save you from a much more expensive repair.

I just greased everything maybe that’s what you are seeing. I just changed the differential oil and had quite the time getting in, am replacing the bolt today. Is that what the breather is?

Probably was the grease I'm seeing. Diff breather looks like this when removed:

It has a little cap that can lift slightly to allow pressure to escape the diff. In the rust belt that little cap can rust in place and keep the breather from being able to vent. It screws into the top of the diff, just give the cap a wiggle and see if it can move around. If it's rusted shut you'll want to replace it.

    Ok yes thank you @CrustyTaco

It had rusted shut, will be replacing today. I’m pretty sure the previous owners had never even changed the differential oil before! Phew this truck definitely needs some TLC
