There was a thread about this a little while back. The consensus seemed to be that he was on an extreme incline at low RPM and it caused a drop in oil pressure which broke something internally.

Don't most professional off-road trucks run dry sump oil systems?

I have done a bit of research on this issue. I spoke with Marc from Frankenstein Motorworks (He is a specialist on the 2GR series engines and the like). He and his group of peers had a good look at the internals of this engine and claimed that it is not oil starvation but the crank case ventilation system swallowing a big slug of oil and hydro-locking cylinders #5 and #6. A large air-oil separator (sealed system) on the PVC breather system placed as far forward in the engine bay will likely solve this issue.
