shouldnt piston slap be present on the cylinder walls if using a inspection camera down the spark plug hole? you could just have a block thats out of spec enough to cause this

are you saying piston slap is a condition that gets worse over time? It ususally does just appear

Yes, there should be indication of piston slap by scoping the cylinders... but piston slap is a cold engine condition, OP's noise is at hot start.

    UPDATE:
Since the first resealing of the timing cover leaked almost immediately after the repair, the truck went back to the dealer for a second try. A re-reseal of the timing cover you can say. The service writer added the knocking sound to the service invoice. I went as far as to provide the service writer with a thumb drive of the video so that the tech could hear the knocking noise. My truck was under the knife for a week before it was returned to me. When I took possession of my truck I met with the tech that performed the reseal. He advised that he and all of the other techs listened to the knocking sound captured on my video. He told me that none of the techs have ever heard that noise from a healthy oil filled 3.5. The tech advised that he carefully examined all of the internal parts that he could see during the reseal and all of the internal parts that might be effected by a reseal repair. He said the internal parts of the engine look perfect. I'll add that since the second reseal of the timing cover the engine has never made that knocking noise again.

The cause of the engine knocking or slapping noise remains a mystery.

Another words, the timing cover leak was found, and hopefully fixed but your back where you started as far as the knocking issue? I can tell you i've heard some of these 3.5 engines in our water companies fleet of 3rd gen tacoma's....some just make more noise like that than others, yet these trucks are on the road all day.

Ive had some hydraulic lifters that sounded like that. If they werent pumping up with oil, or getting old they would tap pretty loud being loose and slapping between the cam and the valve.
