    2009 TRD Sport 4x4 DCLB 205000 miles. Feels like my transmission is slipping around 45 MPH especially going up a hill. Seems to slip and catch repeatedly until I accelerate through it. Also noticed the truck seems to roll quite a bit while in park if not parked on completely flat ground. Any advice?

    Have you checked your transmission fluid level and condition?

    Ha funny story I’m actually sitting in the urgent care waiting on stitches from breaking loose the fill plug bolt. The drain and overflow bolts are soaking in PB blaster. Could that potentially solve the issue?!? Seems too good to be true.

    It could be as simple as low fluid, it's definitely the first thing to check when you have a transmission complaint.
    Remember the engine must be idling in park and the fluid temp needs to be 97-115F to properly check it.

    My 06 had the same shudder at 45 mph. A trans fluid change fixed that. 5k mikes later, would not shift above 3rd gear. New solenoid and it is running great again. Hopefully no more trans issues for a while.

    i should have gotten stitches when i did mine. sliced my finger a good bit on the heat shield.

    Having low fluid may sound simple but it isn't. First not simple to check without a dipstick, not simple to top up, again because no dipstick and fill tube... and more important if it is low that means you have a leak you will need to find and fix quickly or else you'll be having to keep crawling under to top it up. I had such a leak which in my case was a rusty trans cooler line that developed a pinhole leak and caused symptoms similar to what the OP describe.
