    I want to buy a bottom-of-the-line access cab 4 cyl. SR Tacoma, but reeeealy would like to have a manual trans. Is it possible to special order a truck like that from a dealer?

    No. Toyota has their combinations of options they offer and you can take it or leave it.

    Even when your “order” through a dealer they’re just calling dibs on that combo of options when it comes off the assembly line. They don’t custom make it to your order.

    You have to buy what ever TRD that's available with a manual if you want a manual. If you want an access cab it's a going to be a Sport in the USA or an OR in Canada. All other AC are autos. Some buyers have settled for a TRD Pro as it was the only manual they could find.
