Actuator Supply / Throttle/Pedal Position / Brake Override Malfunction

    Rebuilding a 2018 Tacoma.

    The car is starting like a champ now (wasn't an easy journey to get it there) yet running in a "safe" mode where RPMs only rev up to 2 when I fully press the gas pedal.

    I am getting the following errors on a generic OBDII scanner:
    P0658 - Actuator Supply Voltage 'A' circuit low
    P0123 Throttle/pedal position sensor/switch 'A' circuit high
    P2135 Throttle/pedal position sensor/switch 'A'/'B' voltage correlation

    techstream scan gives this error
    P065714 Actuator Supply Voltage 'A' circuit short to ground or open

    the instrument cluster shows: 'brake override malfunction visit your dealer' message.

    From what I understand this has to do with throttle body / flow meter / pedal sensor and overall circuitry between them and ECU and because of incorrect voltage numbers or short throws the car into this 'safe/snail' mode. The brake override malfunction could be part of this as well or a separate thing.

    Here's what I've done. Rebuilding the truck. it wasn't starting. I put a new engine, transmission, starter, alternator, put a new working ECU, programmed the VIN and paired them up, the truck wasnt starting. then put the new throttle body and flow sensor and that got the truck to start up and the engine to run smoothly - however only in this "safe" low rpm mode. Have replaced the accelerator pedal as well - still the same. Fuses are all checked and good.

    not sure what else could it be or where and what for I should check?
    any specific wires I should check? what is this exact circuit A?
    any specific ECU reprogram/flash is needed to reset this?

    before I take it to the dealership

    not sure. i have both the original one and the replacement one and it has the same issue - so don't think it's the pedal, unless there's some sort of reset sequence that needs to be done anyway. i thought maybe there was also reset sequence for brake override malfunction. (not that it matters but also flushed, replaced all the brakes and mechanically they work great now)

    Curious if you found out what the issue was? I was off-roading and got back home to find my neutral safety switch was disconnected. Reconnected the switch but now my truck is throwing the same codes that you were experiencing. Any tips?
