I don't think it will be the three toed sloth or the naked mole rat.

 

I've tried Caribou and Hippopotamus, so Porcupine is the last in that list with 4 different vowels......unless she meant just 4 vowels not necessarily different ones.

Of these 3, I'd like to have Hippopotamus as a last name least.

 

Since it only sounds like the animal but isn't necessarily, ya gotta start thinking of famous names that slightly resemble these creatures.

So my next guess is Hippocrates! He was a Greek physician after all!
