    With out riding in the truck, I don’t really have any more ideas?

    Unfortunately things like this sometimes can even be difficult to diagnose even in person unless you find someone that has good knowledge in reading and interpreting scan data and at least a basic knowledge of how to use a scope. It's unfortunate there's so many shops that use the parts cannon approach to fixing things, there's many good shops too but the trick is finding a good one.
    All we can do is give you things to check into based on symptoms, ultimately you have to decide to try to diagnose it yourself, pay someone to diagnose it, or fire parts at it (not recommended).

    I would get closer to 17....but-I was living down in town on flat ground now I drive up and down a mountain 3 times a day....didn't think of that part.

    Mine are original with 350k on them.

    Have you considered a cracked spark plug insulator?

    An easy test, since you have an auto. With foot on the brake pedal, trans in drive, give it a little gas pedal to load the engine. See if it starts misfiring.

    Next, what grade gas did you use, and did you get it from a top tier, or something else?

    It's not that since I replaced with brand new denso plugs.

    I am unable to get a misfire with the vehicle stationary and brake pedal applied as well as gas pedal.
    It needs to be in motion. I haven't checked carrier bearing or driveline.
    I use 87 octane standard. I read about this being related to weak spark so I increased volatility of the fuel Im burning by using 93 on a full tank, still the problem persists.

    I am going to go with Fuel pump/filter related.

    Also check all vacuum hose ends in the engine bay. Looking for splits and cracks.

    Same symptoms with two sets of plugs. I know it's super hard to figure out online. I don't like playing parts mechanic either.

    any conclusion on this? fighting the same issue. 2005 with 255,000. replaced all coil packs, spark plugs, cleaned throttle body and maf sensor..... still does it.

    One dude on here had that issue with a VVTI solenoid. Everyone thought transmission but it ended up being that. Idk possibly check those

    Mine currently is doing what yours is at the 35-40mph zone. Only up inclines when i have to stay in it behind someone at part throttle to keep speed. I seen that post about the vvt solenoids. I want to try that for myself. Mine only does it at that speed. You would think a shutter would be at multiple speeds. I could be wrong though. Honestly, makes me really wonder to if part of it is just toyotas shift points. Ive seen mine in full lockup today as low as 30.
