Shift Linkage Cable Disconnected

I have a 2022 Off-Road, about 50k miles. Recently it was refusing to get into park, the shifter would move into park, but the dashboard and lights wouldn't show it in park, and it wouldn't turn off cleanly. I found the shift linkage cable was disconnected from the transmission because the nut holding it on fell off. Photo shows the linkage reconnected, but the nut is missing.

If anyone else has this issue, I found out that the nut is https://autoparts.toyota101.com/products/product/nut-flange-9008017229. Curious if people have heard of this happening before? I guess it wasn't torqued properly in the factory. Seems easy enough to fix, but makes me wonder what else will go wrong with the truck in the future...

i ask him which side so i could check mine and he replied drivers side.
strange, never heard of this then 2 different posts in a week.

Seems worth checking, but OP of the one two weeks ago did admit he recently had transmission pulled by shop to replace rear main seal.

 

Just put on a new flange nut with blue thread locker and forget about it.

 

I've never had anything but routine maintenance done on the truck. Ended up being an easy fix, just a bit annoying / concerning it would come from the factory with this issue.

I have no idea what you are talking about?

I know that if the nut bottoms out on the shift linkage, it will come loose again.
Maybe the linkage wasn't on the stud/bolt all the way from the factory.
If so, the nut will bottom out on the linkage. And anytime you move the linkage, it will "turn" the nut.

Oh, yeah.
Things happen. Sometimes at the factory, other times by someone else.
Sometimes they never get put on in the first place - I've done that

 

Had this issue on a 2005. Toyota forgot to torque the nut when they replaced the frame. Easy fix. Wouldn't worry about anything else in your case, likely just a fluke.
