Weathertech floor liners moded to hold OEM floor mats, weathertech vent visors, Toyota bed mat, LEER 100XL cap, hood gasket mod to stop whistling.
Check your blower resistor plug for burn marks, that can surely cause intermittent fan. You may even want to turn AC off, put the fan ON, and wiggle the connector on the blower resistor. It could even be a bad motor but replacing the resistor next is a cheaper fix.

Give it a little time for the resister to cool down when/after after this happens. If that resister is still hot when you grab it (or any of the other connectors) it will give your fingers a good burn.

Resistor isn't in play on high speed, ground goes directly from the switch to the blower motor on high.
