They’re on the signals. One tapped into the ground and one into the positive.

Is that how the instructions said to do it? Without seeing exactly how you have them hooked up, it doesn't sound like what you have is right. If you have them hooked up the way I'm thinking, you're making it worse.

The way you want them hooked up is cutting the positive wire and hook one end of the positive wire to one end on the resistor and the other end of the positive wire to the other end of the resistor...in other words, in line...to add resistance. One on each signal.

When @caribe makaira says to swap the adapter cables from one side to the other, that will ever so slightly change the resistance to the signals. No matter how you try, every adapter cable will vary in resistance and sometimes, all it may take is a couple of Ohms of resistance to change your problem.

OP, you have them as they should go.

With bikes (much of my wiring experience over the past 25 years), you could replace the flasher relay. What I understand with the Tacoma is that that isn't possible and you need the resistors inline to imitate the resistance of an incandescent bulb. If they are added between the positive and negative leads, you're actually reducing the effective resistance.

I wish we could see what you have...

I followed this video (unless it's supposed to be done differently for the front signal versus the rear).

I put the original Diode Dynamics one back on yesterday. This is how it's currently installed.

Are you saying both ends of the resistor should be on the positive wire?

OP, you're fine in the resistor install.

Quick update. I heard back from a rep at Form Lighting. I shared all the troubleshooting mentioned here, and they are just going to send a replacement headlight for the driver's side. If that still doesn't fix it, I may just go back to OEM headlights and sell these unfortunately. The only thing I wish I had a chance to try would have been swapping this headlight onto a Taco with LED DRLs to see if they have any issues or if it's strictly because I have an SR5.

Again, I do appreciate all the suggestions! Fingers crossed that a new headlight is the answer :fingerscrossed:

OP, did you swap the short harness?

I never got around to trying that. Although I’m not sure I understood which one it was, because I don’t recall seeing any that could be completely removed other than the extra one they provided.

My replacement headlight should arrive tomorrow though, so I plan on installing it this weekend.

caribe makaira I did end up seeing which harness you were talking about. However, I installed the new headlight today and everything is working as intended. Huge shoutout to Adam at Form Lighting for helping me with the whole process! Great customer service goes a long way.

I had this on my old truck and it was the bulb not the fuse. When bulb is burnt, other spare bulb starts working.

I've had the same issue with previous vehicles too. All the bulbs in the old housing checked good. I'm guessing it was just something in the wiring. Just glad it's working how it should again
