Appreciate the suggestions but the truck is gone now. Bought a 2021 TRDOR

Nobody mentioned the elephant… er, son in the room. Does he 4-wheel a lot? Any stream or puddle crossings? If so he’s lucky, accidentally hydrolocking an engine will cost thousands to fix.

    FYI

The Toyota Tacoma's 3.5L engine utilizes a system called "D-4S" which combines both port injection and direct injection, meaning it can inject fuel into the combustion chamber directly through injectors as well as into the intake port depending on the engine load and driving conditions; essentially using both methods simultaneously to optimize fuel efficiency and power delivery.

I missed a word, fuel does not "only" get introduced in the intake manifold. Thank you.

I don’t understand you. How does direct injection introduce another way for water to enter the cylinder?

The Tech misspoke, he meant Octopus, not water. An octopus was in the cylinder, it must have squeezed down along the sparkplug threads and fouled it out.
    Happens all the time.

Because water can be in the fuel tank and sent to the fuel rail and injected in the cylinder via the injector.

It's not going to show up as a puddle on top of the piston though. Fuel enters the cylinders as a mist or vapor. The pressure in the rail and the relative vacuum in the intake/cylinder makes it vaporize as it enters

Also if there was that much water in the fuel tank the engine wouldn't run at all

The vehicle is gone anyway. The shop was dragging their feet and refusing to call it what it is.. a slow leak in a headgasket. They either didn't want to do the work or they didn't know how..

Pulling a spark plug and allowing water or debris to fall down in the cylinder is a total rookie move and a legit shop should be embarrassed to let that happen

I was not including fuel as likely way to get water into the cylinder, but I suppose the chance is higher than it leaking past the spark plug. However, the engine needs to be running for water to get into the cylinder via fuel, thus it will be immediately turned to steam and ejected via the exhaust.
