'04 DCSB 3.4L, P2121 & P2138 help

Driving home the car goes in to limp mode. Got the car home, hooked it up to the scanner, and got the following codes, P2121 and P2138.

Did some google searching to find some saying it might be the TPS. Asked my mechanic friend what it could be and he said he'd have to diagnose it to be for sure but what exactly is going on.

Does anyone have any ideas? I was going to order the TPS on RA but they only have the 3-pin TPS, not the 4-pin which is what my car has.

I would start with cleaning connectors and see if that doesn't clear things up.
