Take it to a machine shop for a full disassemble and rebuild.

The question is more should I clean and break loose the carbon on this engine?

If you are intent on going for it run a qt. of transmission fluild thru it just before the 1st flush/oil change then some more gental product snake oil additive for a while. Just watch the oil pressure very closely.

Be ready to pull the pan for a plugged oil pickup.

Holy fuck, have you pulled a spark plug to see how the cylinder looks, how does it run?

Lots of oil changes using high quality full synthetic oil and a can of BG MOA and Napa gold oil filter.
First 3 oil changes need to be oil and filter at 50 mile intervals.
Drop the oil pan to remove larger carbon debris and make sure the pickup tube is clear.
After that just change the oil filter at 50 mile intervals and the oil at 100 mile intervals till you've hit 1000 miles.
You are going to need to be prepared for multiple oil leaks and possibly cam phaser and actuator replacements.

Honestly this may be one of those cases where you drain the oil, only fill half with new oil and the rest with kerosene, let it idle till it gets to operating temp but do not rev the engine, pull the valve covers and oil pan, wash it down with kerosene and hope for the best once its all back together. Pretty hard for it to get worse, right?

Or just replace the engine now since even after all that the engine may still never be quite right without a complete teardown.

    wtf!!!!!!!
    how many miles?
what are you chasing to remove the valve cover to have look...
You may be dealing with a rod knock & cleaning will do nothing....
It has death written all over it...

cut open the oil filter & look at both sides of the pleats for metal debris...
Take a few rear cam caps off, if the top cap isnt mirror smooth & theyre grooved, its dead...

this is what we used to do with a really sludge'd engine. but like Joeyv141 states, don't rev the engine at all. You'll run the risk of spinning a bearing. Like others have stated, get a bore scope and have a look. take off a couple of cam caps and see what the surface underneath looks like. Keep us posted on what you do and the results you get.

I thought this was a very good video. I don't understand all of the science that goes into this product, but Valvoline has been making great motor oil for years. what I haven't found yet, is do you just run it like your regular motor oil and change when your Maintenance schedule advises.

Many companies sell an internal engine cleaner. I'm partial to Amsoil, but ATF also works very well, and so too other brands.

Regardless of which brand you use - be sure to first install a cheap full flow oil filter. You have to provide someplace to collect the debris you break free and clean out.
If you use the already used existing filter, it is very likely to go into "bypass mode" and circulate the debris right back into your oil system. = not good.

Warm up engine, drain a quart out of your oil, replace oil filter with inexpensive one, add engine flush (adds back the quart you drained), and allow it to circulate by idle ONLY. Do not drive it or rev it up. 15 minutes later drain everything out and remove the oil filter.
