I have recently been on the hunt for the cause of excessive exhaust smoke, as well as subtle misfires upon cold starts. my 5vzfe has 325k miles, I recently did plugs, wires, coolant system flush, radiator cap, pcv valve. truck runs amazing, but the slightly sweet exhaust, extra smoke and the very inconsistent misfires has set me to dive deeper. I got the truck in winter, and have yet to see it run in warmer weathers so initially didn't think much of the extra smoke until its been warmer out.

I have gotten no CEL but misfires are noticable for about 10 seconds or so upon half (rough estimate) of cold morning starts in the past 2 weeks or so. The truck doesn't seem to want to do it all day after the first start regardless of how long it sits, only overnight in the morning starts.

The exhaust once driving and fully warm looks normal. here is a video of me blipping the throttle. The engine had been running for 5 minutes or so, Not thermostat open warm, but no longer cold. I did park the truck in the garage to see if it was condensation but clearly shes still huffing and puffing as shown https://youtube.com/shorts/_HK1zxFugNk?si=Rfy90H3yrWOYafpJ

When it comes to head gaskets and/or a cracked head, I have no symptoms other than what's stated. No milky oil or cap, no major coolant loss if any at all. I performed a block test with the weird liquid that changes color. after trying it for 5 minutes while the engine was warm, and this morning with the engine cold I no color change meaning i "passed" the test. The engine hasn't been overheated, temperatures sit at 187. (the smell of the exhaust doesn't smell like fuel, but it isn't obviously coolant. it is sweet, but me and a few others have just said it smells like old 90s truck.

Im really confused at this point. Im now thinking it could be something with fuel sys or intake. if anyone can come up with anything else, please let me know. I really hope I'm not chasing my tail around in circles with this one.

Might be a cracked head, between the valves and spark plugs. Serviced a Tacoma a few years ago that will show little to mild smoke during cold start and nothing once the engine heats up. This was during the summer, shouldn’t be smoking. What confused me at first was it held compression and passed. Passed the leak down test too, shows no exhaust gas in the cooling system. Decided to remove the heat to get the heads pressure test and cleaned. Before cleaning, nothing was noticeable.

Reman head installed, fixed the issue.

Couple of things come to mind, the first is ASAP and when it is safe, take the pressure off of the cooling system and let her sit overnight and see if it changes the next is to pressureize the system with a cooling system pressure tester and see if the pressure drops and/or keep pressure on the system over night and see what happens in the morning.

The issue as I see it is the mileage, who knows what you are going to find once you open her up, corroded heads, cracks or just a failed head gasket... cmon toyota, cant you make a truck that will last more than 30 years!
