I have a 2009 Toyota Tacoma 4.0 V6. I was just wondering if anyone has ever mistaken intake manifold gasket leak for a blown head gasket ? The miles on mine are 183k. Coolant was found in the cylinder but not sure which of the two is the reason. I’m going to do a tube test for the coolant to see if there’s combustion gases in it.

Not sure how you could, there's no coolant running through the intake on the 4.0 so any coolant getting in a cylinder would have to come from the headgasket or a crack in the head itself.

I've never seen a throttle body leak internally.

Should be pretty easy to confirm a headgasket with a borescope and pressure on the cooling system, you can see the coolant dripping in the cylinder if there's a leak.

Only way for coolant in the cylinder is through the cylinder head, block, or the gasket that separates them.

Intakes with crossover coolant channels are gone with the dodo.

There would likely be standing water (coolant) in the intake if anything upstream of the cylinders was leaking
