I finalized a warranty purchase with Jerry Johnson at MidWest Toyota. This is a paste of the email I received form him post purchase.

Working with Jerry was a pleasure, he is straight forward, no games. Unfortunately it seems I missed a year end (2011) $100.00 discount, but still a great price. THANKS JERRY.
