lift, yokahama geolanders,

I have an 01 dc 4x4. i have only owned the truck for about a month and I have been noticing a chatter from the engine, that seems to follow the throttle increase and decrease. Is this lifter noise or possibly injectors? Are the lifter bodies adjustable? It also is still shifting hard> An recommendations would be great.
