Just noticed that the driver side of the rear axle is sitting approx half inch forward of the passenger side, measured from axle to rear leaf mount. I suspect the shop that installed my lift is responsible. Is this cause for concern other than increased wear on the tires?

Are you running lift blocks? The same thing happened to me because my blocks moved

My Chevy 3/4 ton was out 1/4" and it made the rear track 3"-4" off from the front. I discovered this while following my wife taking the truck to get her horse. I notice the steering wheel was turned slightly while drive straight also. Took it to a auto body shop, fixed for $120.

Mine did the same thing but was out an inch the day I purchased it so I had to take it back to the dealer to get it fixed. They said the blocks moved.
As to causing other issues is can move even more so it can increase the chance of skidding on a slippery surface since the vehicle is not tracking straight.

    No blocks. OME Dakars.

Sounds like a good idea to just take it in and fix before it possibly gets worse.

    maybe check the u-bolt torque?

have axle shims installed? If you didn't extend the collar pins, it may have shifted in the slot.

Its also possible that the bed itself isn't straight and square, causing the difference in fender gaps. Or someone installed a spring pack backwards. Get it corrected ASAP since there are possible safety risks.

Edit, just saw you measured to the hanger, so that eliminates the bed.

Is there an easy way to tell if one of the packs is reversed?

The "-", "0", or "+" should be forward.
