The explanation is a little vague I would leave the back wheels on the ground jack the front up put it in FWD and see if I could turn the front drive shaft if you can then yeah there is a case issue. They are pretty tough transfer cases personally I would want to remove any doubt before buying a case.

I think he pretty much has it there. If his hubs are engaged, and the transfer case is in 4WD, then the front driveshaft should turn. Heck, the driveshaft should turn in 4WD even WITHOUT the hubs engaged. Something is definitely up with the transfer case.

OP, have you had ANY transmission, or clutch work done recently?

But with all 4 wheels up off the ground in 4wd I can spin the wheels with hubs locked and it spins the axle and driveshaft

Yes, and since you see the driveshaft turn by spinning the wheels, then you are most likely right that the fault lies in the transfer case. The question is if the problem is internal, or in the shifter. Only way to know that is to pull the shifter covers, and pull the shifter to take a peek inside the opening and look for problems. I have seen shifters incorrectly installed that would cause identical problems.

When I talked to the Toyota parts department they had mentioned "moon" keys or keepers is what I call them. Could be sheered on one the shafts gears internally. Which the truck just rolled over 200k. i think it would have had to be beat on for the entire 200k miles for something like that to happen I'm not having any abnormal noise out of TC that's why I'm leaning towards a fork issue but then again I guess stranger things have happened
