Runs rough, good idle?

    '98 3.4L, 5 speed, 220K, running up the highway today I noticed during acceleration that it was running very rough; shaking the truck rough. Idles fine and when off the gas it runs fine. But holy wow, got 'er up to 70 and it felt like I doing 90 by the sound/feel. Replaced the fuel filter, fuel pump is 3 months old, and am going to clean the MAF tomorrow. What would cause this rough acceleration?
