My wife got a new 2009 Fortuner(SUV using the Hilux chassis and same motors)
and the V6 jerks and splutters(almost like a misfire or fuel starvation) when cold and is even slightly noticable when hot, it has been back twice and they seem clueless but say they need it for longer to check out properly. Any ideas???

Take it back to the dealer.... You should still be under warrantee!!

(unless they do things differently there in Africa)

Seriously - if you start messing with it to try and fix it yourself, the dealer could use that againest you. Atleast, that's what they try to do here in USA.
