my truck a 1st Gen Crew Cab Tacoma 3.4L 4x4, is in need of at least muffler back or Cat back exhaust, Its rusted right after the muffler according to my Mechanic(Uncle).

I do some mild off roading. But I live in Maryland, so rust can plague anything I put on it.

I put a Gibson Catback system on back in 2010, and its already rusted. So I'm leery of buying another one. Any ideas on what I should consider?

Been there done that.... Aftermarket catback exhaust setups don't last very long if they're not stainless. I had a Flowmaster that rusted out in 4 years.

Get a stainless aftermarket system - if you're willing to swallow the cost.

    OR

Find an exhaust shop and have them custom bend a setup for you

    OR

I don't know how realistic it'll be to find 'used' OEM for your truck.... but worth a research. I had my 2007 catback OEM setup in my basement for 4 years until I got rid of it. There could be some folks who have an OEM sitting in their basement.
