I'm no mechanic/technician but fuel delivery from the very first post was my thought. Maybe injectors/coils? Fuel filter was another one of my thoughts. Vehicles are designed these day's for a "one shot" deal. Once they start wearing out it's too expensive to keep them for the average middle class American. There is reasoning behind it I won't go into. Luckily, mine runs at 168,000+ pretty much like new but I go beyond normal maintenance to make that happen. Top Tier gasoline is one of those things. I use Amsoil PI every 4000 miles like they say and although I run deathanol free gas and I do run Chevron deathanol+ for a few tankfuls every year. Chevron is the best gas on the planet because of the "Techron" additive which we can all buy. Cheap gas saves you money short term. Not long term. Out on the east coast there are equally spectacular gasolines such as Chevron. You just have to do the research. I also run SeaFoam in my gas a couple of times a year as well as run SeaFoam through my throttle body once a year. If I remember. I also get oil analysis done at every oil change and I replace air filters every year. I "DO" need to take the throttle body off and inspect it since I haven't done that since I bought the truck with 119,000 miles from my dad.

This thread reminds me that it's time for a new fuel filter.

New Pump and Filter Done. Cleaned a Bunch of Crud out of Tank. Runs Smooth and No Hesitation while Driving it the Little bit that I Did. I will know for sure tomorrow when I'm in traffic. If I got it to a 1/4 tank it would Hesitate like Crazy. I was shutting it down at every stoplight. No fun when it's 110 outside. I hope I can close this bullshit chapter once and for all.

Rinsed it out and put some Rust Dissolver in at Full Strength for about a Half hour or so. Sloshed it all around. Then filled it up with water for another half hour or so to sit with dissolver diluted. Drained then rinsed out again really good. Sat and let dry out. A lot of Shit came out for Sure. Runs like it did when I got it. Stoked its Fixed.

This is awesome. I've been following this thread for quite a while now and am glad your problem has been fixed. I would probably have nuked my taco going through everything you did

So you had replaced the fuel pump once before right? So I guess the issue was crud in the tank then?

Correct. When I replaced the fuel pump the first time I did not drop the tank and clean it out. I picked up the bed moved it back and changed the pump that way. In the matter of about four months the hesitation started.
